1.General job duties. Types of procedures required. Will treat patients in the home setting doing Initial evaluations, routine visits, reassessments, and discharges.
2. Equipment utilized in the unit/dept. (Is there computerized charting, medication systems, etc.) Electronic Medical Record using Epic
3. Required skills and minimum years of experience. >1 year ST experience;Home Health Experience Preferred;OASIS experience preferred
4.Specific licensures and certifications/registrations required. (ARRT, CRT, BLS, ACLS, etc) BLS;Illinois License for Speech Language Pathology
5.Shift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?) 1st shift. Monday-Friday 8-1630. Will work independently going to patient s homes. No on call, no weekends.
6. Floating to other units/departments. (If so, to which units/departments?) No
7.Dress Code: Scrubs or Business Casual
8.Tell us about the unit/department specific training and orientation program. Online and 1: 1 in person training for a couple weeks
