Health Advocates Network is currently seeking a Social Worker to work at a facility in Albany, NY. These are registry positions with our company. *$100 Sign-On Bonus*

Pay Rate: $42 / hour

Shift details:

• M-F: 8am - 5pm

Responsibilities:

• Assist in the implementation of the new Article 29-EE PACE Program,
• Assist in the development of the application materials to ensure the inclusion of the current Article 44 requirements,
• Provide information regarding the application process with the Centers for Medicare and Medicaid Services (CMS),
• Assist with the development of policies and procedures for the PACE program,
• Draft replies to inquiries about the PACE Program,
• Assist in the development of tools, training, reports and other materials for the PACE Program,
• Handle any other ad hoc assignments as needed

Minimum Qualifications:

Licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) OR must possess a Master's degree and one year experience in Social Work (this can include advocacy or administration in a health care facility setting). In addition, the candidate must have strong interpersonal skills and ability to communicate effectively with internal and external stakeholders;working knowledge of Microsoft Office products;experience with complex data entry systems;strong problem solving and decision-making skills;the ability to manage competing priorities and meet deadlines.

Preferred Qualifications:

• Previous experience working for the NYS Department of Health or the Centers for Medicare and Medicaid Services.
