Job Description

1. General job duties. Types of procedures required.
Performs examination and evaluation of patients' physical and functional status including musculoskeletal, neurological, cardiopulmonary and integumentary systems as they relate to functional mobility skills. Includes gathering information from the patient chart, patient and/or family, or other source as appropriate. Analyze evaluations results, establishes PT diagnosis/prognosis,, sets appropriate short and long term goals, and designs an individualized plan of care. Selects, administers, and monitors treatment interventions as appropriate. Reassesses patients' responses and/or outcomes to intervention, adjusts program accordingly, and coordinates treatment with patients, families, physical therapist assistants, and other health care disciplines. Communicates treatment changes to physician as appropriate. Recommends otehr disciplines, programs, agencies and equipment to patient and/or family, as appropriate. Plans and documents appropriate home programs. Instructs patients and patient's families in appropriate continued exercise or therapy programs and in use of therapy techniques and assistive devices. Collaborates with interdisciplinary team to improve quality of care and facilitate discharge planning. Maintains personal productivity at or above department standard. Supervises, orients and evaluates students, orients and trains new department staff and volunteers as assigned. Maintains accurate and timely documentation including evaluations, re-evaluations, progress notes, discharge summaries, patient education, and prepares documents to communicate to appropriate care team Mission Partners. Attends patient review conferences, clinicals, in-service education classes and meeting as necessary. Attends mandatory department, Medical Center meetings, and other work-related educational programs. Maintains knowledge of current trends and developments in the field and incorporates evidence-based practice into the delivery of care.


2. Equipment utilized in the unit/dept. (Is there computerized charting, medication systems, etc.)
Ultrasound, Estim units, treadmills, recumbent steppers, UBE s, Epic EMR, iontophoresis


3. Required skills and minimum years of experience.
Bachelors, Masters or Doctorate in Physical Therapy. Active IL license,
1-2 years experience

4. Specific licensures and certifications/registrations required. (ARRT, CRT, BLS, ACLS, etc)
BLS, Bachelor/Masters/Doctorate in Physical Therapy, state licensure


5. Shift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?)
M-F outpatient 0830-1700, weekend IP rotation (Saturday you determine hours, every 6th weekend, do get day off during week when working Saturday)

SMMC is an 81 bed acute care facility in Galesburg, IL

khakis and OSF logo wear or business casual;if wearing scrubs, black or khaki


8. Tell us about the unit/department specific training and orientation program.
For a traveler, Epic training will be hands on with the Rehab Center of Expertise. It will be at the elbow training for both IP and OP documentation in order to ensure comfort level with documentation in approximately 2 weeks in order to meet the patient and department need.
There will be orientation to the facility by the manager, as well as to other pieces of the computer system, how to utilize API for payroll, occupational health needs to ensure fit testing as well as other PPE needs and ergonomic assessment if required.
