Job Description

General job duties. Types of procedures required.
Responsible for reviewing referrals to meet criteria for admitting to the Swing beds, coordinating the admission, and ensuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines. The MDS Coordinator identifies resident acuity and communicates needs and expectations to families and responsible parties. This role is responsible for assessments and process for Minimum Data Set (MDS) completion and transmission. The Nurse works with the interdisciplinary multidisciplinary team to ensure care planning guidelines for each patient are followed and compliant with Medicare and quality care requirements. The MDS coordinator is responsible ensuring the facility is getting accurate and maximum reimbursements and the Interdisiplinary team is completing their assessments .


Equipment utilized in the unit/dept. (Is there computerized charting, medication systems, etc.)
The hospital has purchased the Epic long term care module for swing beds which will assist the coordinator in more complete and acurate charting as well as assisting with easier documentation by auto population and guidence. They will have an onsight office


Required skills and minimum years of experience.
At least one year as a full time MDS coordinator with proficiency in MDS 3.0 and understanding of the guidelines for admission to a skilled level of care. Prefer but not required: Completion of a training program or certification as an MDS coordinator . Would prefer staff with Swing bed MDS coordination work but not required.


Specific licensures and certifications/registrations required. (ARRT, CRT, BLS, ACLS, etc)

Shift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?)
Monday through Friday 8 hour shifts, May change to M-TH 10 hour shifts. No nights or weekends.

will start at 8 to 16 with possibility of being 20 in future.

This is a new service line at Asante and we will be doing Epic training and some general orientation. The coordinator needs to be experienced in MDS and Intake.
