Job Description

RN discharge Planner with MS/ICU exp 3 years exp

M-F 8-4: 30pm with a 30 min linch

Discharge planners are responsible for coordinating and managing patient discharge planning activities12. Their duties include:- Assessing patients'needs and developing discharge plans for post-hospital care.
- Collaborating with healthcare team members including physicians, nurses, and social workers.
- Arranging for home healthcare services or equipment as required.
- Providing patients and families with information on discharge plans and available resources.
