Job Description


Health Advocates Network is currently seeking a Care Coordinator/Patient Navigator to work at a Facility in Hampton Bays, NY. These are registry positions with our company.

Pay Rate: $26.59 / hour

Shift:

  • DAYS 8:30A-5P M-F


Job Descriptions:

  • Coordinate patient care and appointments
  • Help patients navigate healthcare services and resources
  • Communicate with patients, families, and healthcare providers
  • Schedule referrals, tests, and follow-up visits
  • Educate patients about treatment plans and available services
  • Monitor patient progress and address care needs
  • Maintain accurate patient records and documentation
  • Assist with insurance, referrals, and care coordination
  • Identify barriers to care and help resolve them
  • Collaborate with healthcare teams to ensure quality patient care
